In light of the current situation with the outbreak of COVID-19, we want be certain that we are taking all necessary precautions to help slow the spread and flatten the curve.  Please self-screen by asking yourself these questions:

3. Have you had any of these symptoms in the last 14 days?

4. Are you currently experiencing fever over 100, difficulty breathing or cough?

If you answered yes to question 3 and/or 4, please call your primary care provider.
